Document Revision History
This form appears when you choose a document in the Master Document List and Select menu item Functions / Revisions. It displays a detailed record of the revision history of the selected document.

The top section of the form displays information about the relevant document. The main section lists it's revision history.
Top Fields :
| Field | Description |
|---|---|
| Number | A number / abbreviation used to identify the document |
| Type | A category of document e.g. procedure, form, letter etc. |
| Title | The title of the document e.g. 'Training procedure' |
| Revision No. | The revision being described in this record |
| Revision Date | The date of the revision being described in this record |
Main Fields (Revision history):

| Details | What was revised |
| Authorised by | Who authorised it |
| Date | When did the revision take place |
| Reason | Why was the revision made |
