Document Revision

This form appears when you select the Edit or New buttons in the Document Revision History form. It enables you to edit the details of a revision or record a new revision.

The top section of the form displays information about the relevant document. The bottom section is for you to enter data.

Top Fields (information only):

Field Description
Document Type A category of document e.g. procedure, form, letter etc.
Number A number / abbreviation used to identify the document
Title The title of the document e.g. 'Training procedure'
Current Revision No. The revision number that is currently listed for the document
Revision No. The revision being described in this record
Revision Date The date of the revision being described in this record

Main Fields (data entry):

Field Description
Details What is revised
Authorised by Who authorised it. Use combo box or pop-up form to select name
Date When did the revision take place. Use calendar tool to pick date.
For entering new revisions, this field defaults to the current system date
Reason Why was the revision made

Close Closes the record
